Creating a Feedback Form
Creating a form has two parts: choosing the form type and template, then building the form. This article covers both at a high level. The next article goes deep on questions.
Choose a type and template
- Go to Learning → Feedback Forms and click Create New Form. A drawer opens with two steps.
- Step 1: Choose Form Type. Pick one:
- Session Feedback: feedback after live classes and training sessions.
- Link to Course: embed the form inside a course module.
- General Survey: gather general feedback anytime, shared by link.
- Step 2: Select a Template. Choose Blank Form to start from scratch, or a ready-made template for your type (for example, Trainer Performance, Course Content, and Overall Satisfaction for session forms; Content Rating, CSAT, and Course Rating for course-linked forms).
- Click Proceed (enabled once you have selected). The builder opens.
The form type is chosen up front and shapes what you can add. For example, Session Rating and Trainer Rating questions are only available on Session Feedback forms.
The builder
The builder has two areas: the form canvas on the left, where your questions appear, and the question palette on the right, where you pick question types to add.
Set the two top-level fields:
- Form Title (required): the form's name, up to 100 characters.
- Description (optional): a short description, up to 1000 characters.
Then add and configure questions from the palette (see Question Types & Building Questions).
Use Preview (top of the builder) at any time to see the form exactly as learners will.
Save your form
The save button reflects what you are doing:
- Save Form when creating.
- Update Form when editing.
Publishing is handled as part of saving; a form can be saved as unpublished while you work on it and published when ready.
The save button is disabled until the form has a title and at least one question. If a required question is left blank, saving shows "Please fill out all the required inputs!".
What happens after saving
- Session forms save and return you to the list.
- Course-linked forms show a confirmation with a Go to Module option, so you can embed the form in a course module.
- General surveys show a confirmation with a shareable link and a Copy Link button.
