Question Types & Building Questions
The heart of a feedback form is its questions. This article covers the question types available, how to add and arrange them, and the settings on each.
Add a question
In the builder, the question palette is on the right. Click a question type (or its + ADD button) to add it to your form. You will see "Form element added successfully", and the new question appears on the canvas ready to edit.
An empty form shows "Add blocks to start" until you add your first question.
Question types
- Session Rating: rate the overall session (session forms only).
- Trainer Rating: rate the trainer's effectiveness (session forms only).
- Single Choice: pick one option from a list.
- Multiple Choice: pick several options from a list.
- Star Rating: a star-based rating.
- Linear Scale: a numeric scale rating.
- Date: a date input.
- Number: a numeric input.
- Short Answer: a single-line text answer.
- Long Answer: a multi-line text answer.
Session Rating and Trainer Rating are available only on Session Feedback forms, and each can be added only once per form. Once added, its palette tile is disabled with a note that a form can have only one of that field.
Per-question settings
Open a question to configure it. Depending on the type, you can set:
- Required: make an answer mandatory.
- Duplicate: copy the question (not available for Session Rating or Trainer Rating).
- Delete: remove the question.
Choice questions (Single / Multiple Choice)
- Add answer options under Options, one per line.
- Use Add option to add more.
- You cannot delete the last remaining option.
- Empty options are removed automatically when you save.
Rating questions (Star / Linear Scale, Session / Trainer Rating)
- Set the Scale to 5 or 10 points (shown as "5 stars"/"10 stars" for star ratings or "5 points"/"10 points" for linear scales).
- Optionally add a Start label and End label to describe the ends of the scale (for example, "Poor" to "Excellent").
Reordering questions
Drag questions by their handle to reorder them on the canvas. The order you set is the order learners see.
Keep forms short. A handful of focused questions (one or two ratings plus a single open-ended question) gets far higher completion than a long form.
