Introduction to Feedback Forms
Feedback forms let you collect structured feedback from learners: session ratings, course reviews, and general surveys. This article explains the types of forms, where they are used, and what you need before using them.
Where feedback forms live
Feedback forms sit under Learning → Feedback Forms (the page is titled "Custom Feedback Forms").
Before you can use feedback forms
[!Important] Feedback Forms is a Prime-plan feature. It is available from the Prime plan and above. On lower plans you will see an upgrade prompt instead of the module.
Your plan includes a limit on how many feedback forms you can create. If you reach it, you will be prompted to contact support to add more.
The three form types
When you create a form, you first choose its type. The type determines where the form can be used:
- Session Feedback: collect feedback after live classes and training sessions. Only this type can include Session Rating and Trainer Rating questions.
- Link to Course: embed a form inside a course module so learners fill it in as part of the content.
- General Survey: gather feedback anytime, shared by a public link, with its own audience and visibility settings.
Where forms are used
Depending on their type, forms attach to different parts of the platform:
- Sessions / live classes: a post-class feedback form (session type).
- Courses: a course review form, and course-linked forms embedded as a module material.
- Events: an event feedback form.
- Mentorships: a feedback form on mentorship.
- Standalone: general surveys shared by link.
Default forms
To save time, you can mark a form as a default:
- Live Class Default: automatically used for new sessions.
- Course Default: used as the course review form.
Setting defaults means you do not have to pick a form every time.
Statuses
- A form is active or archived.
- A form is published or unpublished. New forms start unpublished, and duplicated forms are always unpublished.
Permissions
Feedback forms are permission-gated:
- Manage (create, edit, duplicate): management write access.
- View responses and insights: details read access.
- Archive: management delete access.
Admins and instructors have broad access. Buttons and actions hide when you lack the relevant permission.
